  5. Group/or Employer Name


You can also forward the "Referral Confirmation Email" if you've received one for the client. Please allow (1-2) business day for processing/turnaround.


Claims Payment Submissions:

When submitting a claim for services rendered you must submit a detailed invoice, the first and last page of court documents or other "best evidence" documentation to support the invoiced services (i.e. select correspondence, copies of documents reviewed etc.), and a completed claim form.

If you do not reach live support, please feel free to leave a voice message with the Case ID, reason for your inquiry and your best contact information phone and/or email address.
